On this page of our website you will find forms that our customers will need to fill out for our services including boarding contracts, medication forms, emergency contact forms, and owner and pet information update. 

 

There is a separate boarding contract for dog, cat, and small pets so please do fill out the appropriate boarding contract for your pet(s). If you have multiple types of animals you will have to fill out a contract for them (for example if you have a dog and a cat you will fill out the dog contract and the cat contract).

 

Please keep in mind due to Covid-19 we do now have new procedures including filling out paperwork like boarding contracts online. All boarding reservations are made here on our website on this submit forms online page. Please call 610-944-6468 to schedule a grooming appointment. 

 

Please take the time to fill out the owner and pet form to help us ensure our records are up to date. We ask this of both new and existing customers. Also if you add a new pet to your family we do ask that you fill this out again for the new pet. Thank you. 

 

Below you will also see an emergency contact form. We ask that you do fill this out as well especially if your plan to be boarding your pet(s) with us. Although we do not expect to have an emergency we do still ask that everyone fill out the emergency contact form. 

 

 

There is also a medication form. For those of your boarding your pet(s) with us who have a pet or pets who are on medication please fill out the medication form. Thank you.

A screen shot of the completed form can also be e-mailed if for any reason you are having issues submitting the form, or are unsure if it went through. You could also copy and paste the form into an email to submit as well. 

 

Please make sure all questions are answered - if they are not this could be why your form is unable to be submitted. 

 

Please e-mail us at DandDKennel@AOL.com to submit your pet's up to date vaccination record including rabies certificate. Please provide the actual vaccination record and rabies certificate, as invoice and reminder paperwork do not always have all vaccinations or have the rabies vaccination information. If you do not have a copy that is ok just ask your veterinarian and they should be able to send us a copy. Thank you. 

 

If you do not hear back from us within 24 hours confirming we received your form submission please feel free to e-mail us to see if we received it. 

 

Please take note we do e-mail back from DandDKennel@AOL.com after receiving your form - this includes to confirm your reservation, if details are missing from your reservation, if we have questions about your pet, questions about your special instructions, letting you know if we do not have availability, requesting vaccination record, etc so if you do not see a response from us within about 24 hours from form submission please check your spam folder.

​Owner and pet information form

Please help us keep our records up to date. Whether you are a new or returning customer, or if you have a new pet, it’s important that we have your most current information on file.



We recommend the form be updated each year. Otherwise, this form only needs to be completed if there are any changes or updates—it is not required for every stay. Thank you!


Emergency Contact Form

The emergency contact form is to be updated for each of your pet’s visits with us. Thank you for your cooperation.


You are welcome to list more than one emergency contact. Please ensure that at least one contact is available and able to pick up your pet in the event of an emergency.


If your pet has any pre-existing medical conditions or is a senior in a fragile state, please notify your veterinarian prior to boarding or grooming. This helps ensure your pet is healthy enough for their stay and allows your vet to be prepared in case medical attention is needed while your pet is with us.

Please keep in mind you should have an emergency contact who will be available to pick up your pet from the kennel in the event of emergency.

Medication form

If your pet needs medication, vitamins, supplements, ointment, drops, etc provided there is a fee for each medication administration. Please keep in mind someone has to take the time to make sure you pet is receiving the proper dosage, if it is an ingestible medication/vitamin/supplement that person also needs to watch to ensure that your pet does in fact eat the medication. For example - some pets are on medications for life threatening illnesses so its very important to ensure they do in fact get the proper dosage of the proper medication as needed to prevent heart problems, seizures, etc. 



Please remember to only provide the amount of medication that will be needed for your pet while in our care. We ask that you do not bring an entire bottle of medication if it is not planned to be used. 

It is best to bring medication divided out for each administration in a pill container. Pill containers can be found in different frequencies, for example, daily, AM and PM, or AM, Noon, and PM. 


Please remember if your pet takes medication in something such as a pill pocket, meat, cheese, canned food, peanut butter, etc., please make sure you provide enough of this for each medication administration. We do have a refrigerator, so it is not a problem to refrigerate your pet's food if needed. 



Please do not mix any type of medication in with food without specifying that you are doing this first. 

Please take note that there is still a medication administration fee for medication mixed into food. This is because while many people tell us their dog eats it in their dry food - many do not while in board and we end up finding it remaining in the food bowl which can also result in medication not being taken as directed so it is important we are aware of where the medication is and when it is to be administered as your pet's health is very important to us. 


If your pet were to have a medical emergency while in board the veterinarian will ask if the pet is on any medication, vitamins, supplements, etc so for the health of your pet it is important that we are aware of the pet being on medication, vitamins, or other supplements, the names of what the pet is on, and why so this does not negatively effect the diagnosis or treatment of your pet, nor vet charge for diagnosing a health problem you the owner are already aware of. Please keep in mind when emergencies occur - they are just that, emergencies - so your primary and or preferred veterinarian or animal hospital may not be available to your pet's emergency, meaning an emergency hospital with no history on your pet could be where he/she receives veterinarian care.

Small pet boarding contract

The small pet boarding contract is for small pets, not including dogs and cats - for example, rabbits, guinea pigs. There is a boarding contract specifically for dogs and a boarding contract specifically for cats. 


The boarding contract is to be filled out for each stay.

If you have multiple small animals, will they be staying together or do they need to be kept separated?

You’re welcome to bring items from home to help make your pet more comfortable during their stay. To help ensure that everything you bring is returned, please label all items clearly and provide a list with descriptions of each item (for example: enclosure, carrier, food containers, toys, bedding, leash, etc.).


Please note that we are not responsible for lost items.


We ask that all items brought in are clean. Thank you for your understanding and cooperation!

Cat boarding contract

All cats must be up to date on all vaccinations including, Rabies, FVRCP, and FELV. Proof of vaccinations must be provided and kept on filed. You or your veterinarian can e-mail DandDKennel@AOL.com. Do you understand and agree that your cat is up to date on all vaccinations and that you will provide vaccination record prior to boarding?

You can upload your cat's vaccination record including rabies certificate here, or you or your veterinarian can e-mail DandDKennel@AOL.com.

If you have multiple cats, will they be staying together or do they need to be kept separated?
Would you like to add playtime sessions? All cat playtime sessions are done indoors. A kennel technician will play with the cat during the playtime. Individual cat play time sessions are $5 per session.

Please do not provide any extra food unless you expect to be extending your cat's boarding stay and expect the food to be eaten. Please do not expect to get extra food back it if has been provided as we will assume you want you cat(s) to eat the food you provided assuming they finished their other food you provided. Thank you.

Dog boarding contract

The boarding contract is to be filled out for each stay.

All dogs must be up to date on all vaccinations including Rabies, Da2ppv, Bordatella, and Canine Influenza,. Proof of vaccinations must be provided and kept on file. Do you understand and agree that your dog must be up to date on all vaccinations, and do you agree to provide vaccination records before checking your dog in for boarding?

If we are inspected while your dog is in boarding, and he/she does not have a current rabies vaccination or license, the PA Dog Warden can fine you.

Please provide proof that your dog is currently vaccinated against rabies and licensed. You can upload your dog's vaccination record including rabies certificate here, or you or your veterinarian can e-mail DandDKennel@AOL.com.

All dog boarding enclosures are indoor outdoor enclosures with concrete floors. Both boarding areas are heated or air-conditioned as needed. Walks and playtime sessions in our fenced-in grass yard may be added for an additional fee.
Deluxe dog boarding is in the deluxe area, which is an upscale boarding area featuring glass doors, elevated bowls, chew proof Kuranda dog beds, additional bedding, heated floors, seasonal or holiday decorations. A walk, individual playtime, and group playtime are included. Deluxe boarding is $45 per dog per day.
Basic boarding is in the basic area, which is a basic enclosure. There is block and chain link inbetween each enclosure on the inside and outside. Each enclosure has a chew-proof Kuranda dog bed. No walk or playtime sessions are included with basic boarding; however, they can be added for an additional fee. Basic boarding is $35 per dog per day.

Please select if you would like to schedule activities for your dog and if you permit your dog to participate in any activities. Each activity is $5 per session. All walks & playtimes are done on our property in our fenced in yard.

Please keep in mind walks and play times are done in our fenced in yards which are real grass. Depending on whether conditions can effect whether or not the grass yard is wet and or muddy. Please take note that dogs will not be bathed before they go home unless they are scheduled for bathing or grooming. There is a charge for bathing & grooming services. Please state that you understand this.

Would you like any grooming services for your dog before he/she goes home? Grooming services are typically done the day of or day before the dog goes home.

Grooming charges are based upon the size/weight of dog, coat type and condition, desired haircut, as well as the pets’ behavior. (If you would like a grooming estimate please contact us). Please keep in mind your dog's coat condition (for example matting) and dog's behavior for grooming may effect whether or not we are able to perform your desired groom.

Vaccination

Here at D and D Kennel we do require pets to have current proof of vaccinations.  We do need to keep a copy of your pet's current vaccination record including rabies certificate on file as well as a copy of their license. Please make sure we have your pet's vaccinations record and rabies certificate before coming in for boarding. You or your vet can e-mail us at DandDKennel@AOL.com 

We require that all dogs be up to date on the following vaccinations:

 

  • Rabies

  • Da2ppv (Distemper, Adenovirus Type 1 (Hepatitis), Adenovirus Type 2 (Respiratory Disease), Parainfluenza, and Parvovirus. ) 

  • Bordatella (Kennel Cough)

  • Canine Influenza 

*Please take note Canine Influenza is a two part vaccination. 

 

We require that all cats be up to the date on the following vaccinations:

 

  • Rabies

  • FVRCP (Feline Distemper: Rhinotracheitis, Calicivirus and Panleukopenia)

  • FELV (Feline Leukemia) 

 

 

If your pet is not up to date with all required vaccinations please make sure that they do get them before their stay here at D and D Kennel. Most animal hospitals as well as low cost vaccine clinics typically offer all of our required vaccinations for both dogs and cats. With vaccine clinics these tend to be vaccine only, so no office fee or exam fee, just the vaccine cost.
